=== Medication considerations === Some medications require attention when in a state of ketosis, especially several classes of diabetes medication. SGLT2 inhibitor medications have been associated with cases of euglycemic ketoacidosis – a rare state of high ketones causing a metabolic acidosis with normal blood glucose levels. This usually occurs with missed insulin doses, illness, dehydration or adherence to a low-carbohydrate diet while taking the medication. Additionally, medications used to directly lower blood glucose including insulin and sulfonylureas may cause hypoglycemia if they are not titrated prior to starting a diet that results in ketosis.
=== Setting === The game takes place in an alternate historical timeline, in which Franklin D. Roosevelt is assassinated in 1933, allowing Senator Huey Long to become President of the United States. Under Long, it is implied that Japan did not attack the United States, and did not give them a reason to fight against Germany, leaving the United Kingdom to defend itself from the German forces alone. The Battle of Britain was lost, allowing the Germans to invade and occupy the entire country. Most of the volunteer forces of the Home Guard became complicit in helping the Germans, with only a few attempting to resist. At some point during the occupation, the population of the island town of Wellington Wells did what is initially only alluded to as a "Very Bad Thing" that caused the Germans to voluntarily leave their island, allowing the British citizens there to live free. However, the repercussions of the Very Bad Thing left the citizens with immense anguish and guilt over their actions, leading to the invention of a new hallucinogenic drug called "Joy", which suppresses all unhappy memories and leaves its user in a chemically-induced euphoria, while also brightening how they perceive their environment. However, its many adverse side effects include addiction, short-term memory loss, loss of appetite, nightmarish hallucinations, and being susceptible to manipulation. By the 1960s, Wellington Wells' isolation led to resounding advances in technology, including Tesla-styled weapons, portable power cells, and automated security systems.
Bukele has threatened incarcerated gang members. At the beginning of the crackdown, he tweeted that the government had seized incarcerated gang members' belongings, removed their mattresses, and rationed their food. Bukele also posted a video of prisoners sleeping on floors and complaining about a lack of food and sanitation. He threatened to deprive them of food entirely in April 2022 if the gangs attempted to retaliate against the crackdown, citing rumors about revenge killings. After members of Barrio 18 killed three police officers in Santa Ana in June 2022, Bukele said at a press conference that the gangs were "going to pay dearly" for the "ambush" against the police. The government began destroying gravestones belonging to deceased gang members in November 2022 to prevent them from becoming "shrines" and Bukele compared the gravestone destructions to denazification in post-World War II Germany. He warned Salvadoran parents to keep their children away from gangs, since they would lead to "prison or death". Shortly after the crackdown began, Bukele called for the construction of a new 20,000-inmate prison. He announced the construction in July 2022 of the 40,000-inmate Terrorism Confinement Center (CECOT) in Tecoluca, that would be one of the world's largest prisons. In February 2023, Bukele posted a video on Twitter of him and members of his cabinet touring the prison. It is staffed by 250 police officers and 600 soldiers, and covers 410 acres (170 ha).

Deuterium, 2H (atomic mass 2.014101777844(15) Da), the other stable hydrogen isotope, has one proton and one neutron in its nucleus, called a deuteron. 2H comprises 26–184 ppm (by population, not mass) of hydrogen on Earth; the lower number tends to be found in hydrogen gas and higher enrichment (150 ppm) is typical of seawater. Deuterium on Earth has been enriched with respect to its initial concentration in the Big Bang and outer Solar System (≈ 27 ppm, atom fraction) and older parts of the Milky Way (≈ 23 ppm). Presumably the differential concentration of deuterium in the inner Solar System is due to the lower volatility of deuterium gas and compounds, enriching deuterium fractions in comets and planets exposed to significant heat from the Sun over billions of years of Solar System evolution. Deuterium is not radioactive, and is not a significant toxicity hazard. Water enriched in 2H is called heavy water. Deuterium and its compounds are used as a non-radioactive label in chemical experiments and in solvents for 1H-nuclear magnetic resonance spectroscopy. Heavy water is used as a neutron moderator and coolant for nuclear reactors. Deuterium is also a potential fuel for commercial nuclear fusion.
== Adverse effects == Safety and dosing trials showed that the incidence of adverse events in orforglipron-treated participants was 62–89%, mostly from gastrointestinal discomfort (44–70% with orforglipron, 18% with placebo); in patients experiencing adverse events, 96% had mild to moderate GI side effects, while approximately 4% had severe nausea or vomiting. The most common side effects of orforglipon noted in clinical trials were vomiting, diarrhea, nausea, upset stomach, and constipation.
== Special Operations units == The People's Armed Police maintains several Special Operations Units, also known by Western sources as "Special Police units". They were established in Beijing in early 1980s and in 1983 the first of them was transferred to the People's Armed Police as the People's Armed Police Special Police Group. In 1985 the Group became People's Armed Police Special Police School and, in 2002, it became the People's Armed Police Special Police Academy. In 2002, the Snow Wolf Commando Unit, since 2007 Snow Leopard Commando Unit, was established in Beijing as the second special police unit. According to Joel Wuthnow, the Snow Leopard Commando Unit was moved from the Beijing Corps to the 2nd Mobile Corps in 2018. The special operations units are tasked to carry out counter terrorism missions, riot control, anti-hijacking and bomb disposal. Local special operations units receive Explosive Ordinance Disposal (EOD) training, fast roping, counterinsurgency tactics, along with training in hostage rescue and hostage negotiation.

== Chemistry == Dextromoramide is the right-handed isomer of the moramide molecule. The left-handed molecule is called levomoramide, and a mixture of the two is called racemoramide. Its full chemical name is (+)-1-(3-Methyl-4-morpholino-2,2-diphenylbutyryl)pyrrolidine, and its molecular formula: C25H32N2O2, with an atomic weight of ~392.5. Dextromoramide was discovered during the course of research into a related family of compounds, the α,α-Diphenyl-γ-Dialkyamino-Butyramides, which show no analgesic activity, but are extremely active physiologically as inhibitors of gastric secretions in man. Other drugs from this series show antispasmodic and antihistamine effects, but most research was put into researching analgesics. The structure-activity relationships of this family of drugs was investigated extensively, with dextromoramide representing the optimisation of several different structural features; (i) at the 1-amide group only the pyrrolidine and dimethylamide substituents were active, with pyrrolidine being more potent (ii) the alkyl chain was more potent when methylated, 3-methylation was more potent than 4-methylation, and in the 3-methyl analogues the dextro isomer was more active (iii) while morpholine, dimethylamine, pyrrolidine and piperidine were all active at the 4-amine group, morpholine was the most active (iv) any substitution on the phenyl rings reduces activity.
